When you buy compounded semaglutide or tirzepatide online, a pharmacy you were never told about makes it. Here is every one we have researched, with its licence and its published regulatory record — and no ranking, because there is nothing honest to rank on.
The short answer
- 12 pharmacies researched: 11 still compounding GLP-1s, 1 that have stopped.
- 5 of them have an FDA warning letter on the public record. Read the dates — one concerns a facility the company had already left when the letter issued.
- No compounded GLP-1 is FDA-approved. The agency does not review compounded preparations for safety, effectiveness, potency or purity, whichever pharmacy prepares them.
- A 503B outsourcing facility is FDA-registered, follows cGMP and is routinely FDA-inspected. A 503A pharmacy compounds per-patient under state board oversight. Neither means FDA-approved.
- Your telehealth provider chooses the pharmacy. You can ask which one in writing before you pay, and a provider that will not say has answered.

Which compounding pharmacies make GLP-1 medications?
The pharmacies most often named behind US telehealth GLP-1 prescriptions are Hallandale, Empower, Strive, Olympia, Belmar, ReviveRX, Southend, Optimal Balance, Red Rock, Casa Pharma RX and Epiq Scripts. BPI Labs also did until it stopped compounding GLP-1s in April 2026. The telehealth brand you buy from is not the pharmacy; it chooses one on your behalf.
Which is the best compounding pharmacy for GLP-1s?
Nobody can honestly answer that, including us. The FDA approves finished drug products and does not review compounded preparations for safety, effectiveness, potency or purity — so there is no regulator assessment of the actual product to rank. What can be compared is the registration category, the state licence, and what the FDA or the state board has published. That is what these pages set out, and why they are alphabetical rather than ranked.
Is a 503A or a 503B pharmacy better?
They are different rather than better and worse. A 503A pharmacy compounds against a prescription for a named patient and is regulated by its state board; it is not required to follow FDA manufacturing-quality rules. A 503B outsourcing facility registers with the FDA, must follow cGMP and is subject to routine FDA inspection, and can compound in batches without a patient-specific prescription. Neither route makes the product FDA-approved. The 503B route carries more federal oversight, which is a checkable difference rather than a quality guarantee.
Does an FDA warning letter mean a pharmacy is unsafe?
No. A warning letter is the agency stating its position that it found violations, and it is public and dated. It is not a recall, a criminal charge or a court finding. Several of the letters recorded here concern how a product was described to buyers rather than what was in the vial, and at least one concerns a facility the company had already left by the time the letter issued.
