At the starting dose the FDA-approved product is now the cheaper one.
- No compounded tirzepatide is FDA-approved. The agency does not review compounded preparations for safety, effectiveness, potency or purity — that applies to all six pharmacies equally.
- The shortage that justified mass compounding is over. A 503A pharmacy can still compound for an individual patient with a documented clinical reason; price is not one.
- Ignore "near me". These are mail-order pharmacies shipping nationwide, usually from another state, and distance tells you nothing about quality.
- You can check any pharmacy in about ten minutes: FDA warning-letter database under the legal name, the FDA 503B register, and the state board licence lookup.
The comparison that changed in 2026
Search “cheapest tirzepatide online” and nearly every result answers with a compounded programme. That was right in 2024. It is no longer obviously right, because Eli Lilly now sells Zepbound direct at self-pay prices that overlap the compounded market.

Read the maintenance rows together. Lilly's $449 price is conditional on buying the next fill within 45 days of the last one; miss it and the price is $699. That $250 difference turns on a deadline rather than a dose, and it is the one place where compounded still has a clear price advantage.
Who actually compounds it
Listed alphabetically and not ranked. No regulator has examined the finished product from any of them, so a ranking would be ranking marketing. What can be compared is on the record: registration category, state licence, and what the FDA or the state board has published.

Where compounded tirzepatide stands legally
Mass compounding of tirzepatide copies was permitted largely because the drug was on the FDA shortage list. The FDA declared that shortage resolved and the enforcement-discretion window closed in early 2025. A 503A pharmacy can still compound for an individual patient where there is a documented clinical reason the commercial product does not meet — an allergy to an inactive ingredient, or a dose the approved product does not come in. In April 2026 the FDA proposed excluding tirzepatide from the 503B bulks list; the comment period closed on 29 June 2026 and a final rule is pending. If a programme is selling you compounded tirzepatide on price alone, that is not one of the reasons the exemption exists for.

What matters is the pharmacy’s registration category, its state licence status and its published regulatory record — none of which correlates with distance from you.

Ask in writing before you pay: "Which compounding pharmacy fills my prescription, and is it a 503A or a 503B facility?" Keep the reply.
