In December 2024, the FDA approved Zepbound (tirzepatide) as the first GLP-1 medication with an on-label indication for moderate-to-severe obstructive sleep apnea (OSA) in adults with obesity. The SURMOUNT-OSA trials (NEJM 2024) showed AHI reduction of ~25–29 events/hour at 52 weeks, with 43% reaching OSA resolution thresholds. CPAP remains the gold standard. Other GLP-1s are used off-label.

Off-label considered options
Off-label use: Zepbound is the ONLY GLP-1 with an FDA-approved OSA indication (Dec 2024). Mounjaro = same molecule as Zepbound (tirzepatide) — sometimes prescribed off-label when Zepbound coverage / supply is constrained. Wegovy and Ozempic in OSA are OFF-LABEL.
Compounded alternatives — NOT FDA-approved
Important: Compounded semaglutide and tirzepatide are NOT FDA-approved products. Post-shortage regulatory status remains uncertain. Discuss safety, sourcing, and lab quality with your prescriber before considering.
Non-pharmacologic alternatives
CPAP / BiPAP (gold standard)
Most effective OSA therapy when used consistently. Adherence is the main limitation — many patients can't tolerate the mask long-term.
Oral appliances
Mandibular advancement devices for mild-moderate OSA or CPAP-intolerant patients.
Positional therapy
Side-sleeping devices for positional OSA. Adjunct, not standalone.
Bariatric surgery
For severe obesity (BMI ≥35) — often resolves OSA entirely along with weight loss.
