What the comparisons actually show
- In the only head-to-head trial of the two leading molecules, SURMOUNT-5, tirzepatide beat semaglutide on average weight lost. Both are FDA-approved; which is right for a given person is a clinical question, not a ranking.
- Zepbound and Mounjaro are the same molecule (tirzepatide) with different approved indications — weight management and type 2 diabetes. Wegovy and Ozempic are the same pair for semaglutide.
- Self-pay prices now overlap: Zepbound from $299/month at the starting dose, Wegovy injection $349/month at maintenance, and the oral tablets from $149.
- Compounded versions of either molecule are NOT FDA-approved. The agency does not review them for safety, effectiveness, potency or purity.

Read the percentages carefully.Each figure comes from that drug's own trial programme, in different populations over different durations — they are not a head-to-head ranking, and lining them up as one is the standard error in this genre. Only SURMOUNT-5 compared two of these molecules directly. Drugs marked discontinued left the US market in 2024 and are listed for reference; a brand marked “now …” is still sold, under a new name.
